We commend you on your commitment to a superior education in healthcare in your decision to begin a course of study with the Carrick Institute of Graduate Studies.

Please read the following before continuing with your application:

You are about to complete an enrollment application online. Upon arrival to your first class you will be expected to provide the following documentation:

1. Original (and photocopy) of a proof of identity (e.g. passport, drivers license, student ID card)

2. Photocopies of all diplomas/degrees/board certifications mentioned in your application (students, currently enrolled at a chiropractic college do not need to provide these documents).

3. If you are currently a student at a chiropractic college, you are required to bring a letter from a school official confirming your enrollment and projected graduation date.

Upon submission of this form you will be provided with a temporary Learner ID and password, which will allow you to register for your first course and to view a required reading list, for purposes of preparation.
